Progressive hearing loss and cerebellar ataxia in anti-Ma2-associated autoimmune encephalitis.

چکیده
A 38-year-old Brazilian man presented with a two-year history of progressive bilateral hearing loss, cerebellar ataxia, emotional lability and hypersexuality. A limbic-brainstem syndrome was suspected and neuroimaging studies performed, disclosing marked brainstem signal changes (Figure). Cerebrospinal fluid analysis disclosed a mild protein increase. During paraneoplastic screening, testicular ultrasonography disclosed a heterogeneous complex mass in the right testicle (Figure) and serum anti-Ma2 antibodies were detected. Anti-Ma2-associated encephalitis classically emerges in the context of seminomatous and nonseminomatous testicular tumors and presents with a complex spectrum of neurological manifestations1, including limbic encephalitis, atypical parkinsonism, cerebellar ataxia, brainstem dysfunction, myelopathy, radiculoplexopathy, REM sleep behavior disorder and narcolepsy1,2.

Autoimmune episodic ataxia in patients with anti-CASPR2 antibody-associated encephalitis
OBJECTIVE To report paroxysmal episodes of cerebellar ataxia in a patient with anti-contactin-associated protein-like 2 (CASPR2) antibody-related autoimmune encephalitis and to search for similar paroxysmal ataxia in a cohort of patients with anti-CASPR2 antibody-associated autoimmune encephalitis. METHODS We report a patient with paroxysmal episodes of cerebellar ataxia observed during autoi...

متن کاملAutoimmune encephalitis with GABAB antibodies, thymoma, and GABAB receptor thymic expression
Antibody-mediated autoimmune encephalopathies comprise a group of severe conditions with a varying degree of motor and cognitive symptoms that respond to immunotherapies. The associated antibodies are directed against intracellular targets, such as the classic paraneoplastic autoantigens Hu, Yo, Ri, CV2, Ma2/Ta, or the enzyme GAD, or against cell surface antigens such as receptors and ion chann...
متن کاملAnti-Ma2 antibody related paraneoplastic limbic/brain stem encephalitis associated with breast cancer expressing Ma1, Ma2, and Ma3 mRNAs.
A 69 year old woman presented with cognitive impairment and supranuclear gaze palsy caused by paraneoplastic limbic/brain stem encephalitis associated with atypical medullary breast carcinoma. The cerebrospinal fluid from the patient harboured an anti-neuronal cell antibody against Ma2 antigen, but not against Ma1 or Ma3 antigen. Despite the antibody being restricted to the Ma2 antigen, the pat...
